Summary

The Gonococcal Resistance to Antimicrobials Surveillance Programme (GRASP) monitors trends in antimicrobial resistance in consecutive gonococcal isolates from 26 genitourinary medicine clinics in England and Wales. In 2002, 2204 gonococcal isolates were tested, and the overall prevalence of ciprofloxacin resistance (minimum inhibitory concentration ≥1 mg/L) was 9·8%, compared with 3·1% in 2001 and 21% in 2000. Between 2001 and 2002, prevalence of ciprofloxacin resistance increased two to three-fold, irrespective of recent sexual contact overseas, sex, or residence within or outside of London. These findings suggest that national and local treatment guidelines need to be reviewed urgently.

The main reasons for scoring lower were lack of pointing out the study's objectives or the data sources [24,26,30,36] (Supplementary Fig. 1). The most frequent limitation identified by the studies' authors was not including data on clinical symptoms, sexual practice and treatment [24–30,34–36] and the lack of information of index cases’ contacts [19,20,22,31–33,36]. Antimicrobial resistance to N. gonorrhoeae has become a major global health problem.

And this has caused a potential threat to human health and serious environmental pollution. Among varieties of antibiotics, ciprofloxacin [5] (CIP), as a type quinolones antibacterial agent widely used for infections caused by Gram-negative and Gram-positive bacteria is often detected in wastewater. The CIP is also hard to be removed completely by conventional treatment processes in WWTPs [6].
